The Medical Social Worker provides psychosocial assessments, crisis intervention services and counseling to patients. This position coordinates healthcare services throughout the continuum of care on behalf of the patient and assists in discharge planning. This position requires population specific competencies. * Must be able to communicate effectively in English Social Work Associate: * Requires bachelor's degree in social work. Two (2) years experience in social work preferred. Social Work I: * Requires a master's degree (MSW) in social work from accredited graduate school of social work. * Previous professional experience is preferred. * Experience in a medical setting is preferred. * Must sit for licensure exam as soon as eligible; may work with provisional license for one year maximum. Social Work II: * Requires a master's degree (MSW) in social work from accredited graduate school of social work and licensure from the Oklahoma State Board of Licensed Social Workers (LSW, LCSW, LMSW or LSW-Admin). * Two (2) years of professional experience, post-master's degree, is required. * Experience in a medical setting is preferred. Additional for NZTI only: prefer related experience in a transplant institute. * Hospice and Samaritan Home Health: Valid driver?s license in the state where employed, available and dependable transportation, ability to drive automobile, and proof of current automobile liability insurance.
