Rehabilitation Clinical Leader - OTR/SLP -Trenton MO (378373-240D )

Summary:

The Rehab Clinical Coordinator isresponsible for providing a full range of therapy services including assessmentand treatment planning (Therapists Only), and therapeutic interventions in aninterdisciplinary environment consistent with the position's qualifications,professional practices and ethical standards. This position will alsodemonstrate accountability for and contribution to program development, qualityimprovement, problem solving and patient care enhancements in a flexibleinterdisciplinary fashion.

ESSENTIAL CLINICAL FUNCTIONS:

  • Supports clinicalprogram team supervision and development through onsite treatment observationand feedback

  • Champion identified ClinicalPrograms being implemented with therapy team for application and carryover.

  • Provides dailyclinical leadership to team and responds to any employee exceptions.

  • Requestsdevelopment of team members through structured activities and availableresources.

  • Contributes topositive client relations through onsite clinical support.

  • Participates incare coordination and utilization meetings and activities.

  • Coordinatetreatment activities, goals and discharge plans between disciplines and teammembers.

  • Champion consistent proactiveresident identification culture and activities

  • Conducts initialassessments, provides age appropriate therapeutic interventions and documentsthe course of patient care including progress made and continuing need fortreatment.

  • Contributes tointerdisciplinary care plans.

  • Evaluates patientresponse to treatment and provides feedback to the rest of the team.

  • Provides patientand family education on tasks and resources that will assist with continuedfunctional independence.

ESSENTIALOPERATIONAL/ADMINISTRATIVE FUNCTIONS:

  • Reviews andcompletes Smart alerts daily for assigned sites of service.

  • Supports overallADR/Appeals process.

  • Coordination ofpatient scheduling, daily stand-up meetings, weekly Medicare meetings, clinicalmeetings, admission/discharge coordination, etc.

Knowledge/Skills/Abilities:

  • Communicates and demonstratesa professional image/attitude for patients, families, clients, coworkers andothers.

  • Promotes adherenceto policies and practices of applicable professional organizations, client,Kindred Healthcare and RehabCare Group.

  • Ability tooccasionly travel in the community to meet clients/customers.

  • Frequent andprolonged bending, stooping, crouching walking and standing.

  • Ability tocommunicate.

  • Provision of owntransportation is necessary.

  • Approximate timerequired to travel: Limited

Qualifications

Education:

  • Graduate of four (4) year bachelor degree therapy program preferred or
  • Minimum Requirement, Graduate of two (2) year associate degree therapy program.
Licenses/Certification:
  • Hold an applicable and current license to practice as an evaluating therapist or treating assistant as required per State Practice Act.
Experience:
  • Track record of successful clinical experience and excellence.
  • Experience working with a geriatric population and interdisciplinary team is desirable
